This policy is in place to promote the health and safety of our student-athletes. All students in grades 6th-8th who are planning to try out for a school-sponsored interscholastic or intramural athletic team must have a physical examination performed within 365 days of the try-out date. The Health History Update Questionnaire has to be completed within 90 days of the first day of the try-out date. The Sudden Cardiac Death Sign off Form must be signed and dated. 

A new Health History Update Questionnaire, the Pre-Participation Physical Form, and Sudden Cardiac Death Form is required for each sport season! 

The Student's physical must be filled out on the Athletic Pre-Participation Physical Examination Form and then cleared by the school Physician. The Health History Update Questionnaire must contain the most recent updated medical/ health information. No student will be permitted to try out without meeting these requirements.

Included in the links is the Sudden Cardiac Death Pamphlet. The form that is to be signed states that you have received and read the information in the pamphlet. Please read the pamphlet first.
