Please remember that NO student is allowed to participate in official practice/tryouts until forms are submitted and reviewed for clearance.
Only our school physician has the authority to review and medically clear athletes to participate in sports. What this means is that if you do not have your paperwork (current physical which is good for one year from the physical exam date, and an updated HEALTH HISTORY UPDATE QUESTIONNAIRE FORM submitted by 90 days of the first day of tryouts, you run the real risk of not being able to participate in tryouts on the first day of the practice. The school doctor will be reviewing each and every individual student's paperwork who intends on going out for an interscholastic/intramural sports. You will not be able to hand your paperwork into the nurse's office and practice that afternoon. It could be a week or more before you are cleared, depending on when the school doctor is scheduled to come to George White School to review these documents. Please submit your paperwork ASAP so you are able to participate on the first day of tryouts.
