Please remember that NO student is allowed to participate in official practice/tryouts until forms are submitted and reviewed for clearance.
IMPORTANT INFORMATION!!Only our school physician has the authority to review and medically clear athletes to participate in sports. What this means is that if you do not have your paperwork (current physical which is good for one year from the physical exam date, and an updated HEALTH HISTORY UPDATE QUESTIONNAIRE FORM submitted by 90 days of the first day of tryouts, you run the real risk of not being able to participate in tryouts on the first day of the practice.
The school doctor will be reviewing each and every individual student's paperwork who intends on going out for an interscholastic/intramural sports. You will not be able to hand your paperwork into the nurse's office and practice that afternoon. It could be a week or more before you are cleared, depending on when the school doctor is scheduled to come to George White School to review these documents.Please submit your paperwork ASAP so you are able to participate on the first day of tryouts. Any questions, feel free to stop in and see me.Thank you,School NursePreparticipation Physical Evaluation (PPE) Physical FormThis physical is valid for one full year from the date of the exam.A new copy is not necessary if you have already submitted it to my health office and it is still valid. Pre-Participation Physical Evaluation Forms are to be completed, signed, stamped and dated by the examining physician. The physician MUST sign off that he/she completed the cardiac module. There are 4 pages to the packet which includes:- History Form
- Athlete with Special Needs Form
- Physical Exam Form
- Clearance Form
Physical Exams are valid for ONE YEAR ONLY from the date of the exam. You may not participate with an expired physical. Please be sure the physician signs, stamps and includes the date of physical, and checks off clearance status. Your physician must sign the portion of the physical that states he/she has completed the cardiac module. The physical cannot be accepted if your physician has not signed this or if he/she has not completed this module. Please check this prior to your appointment.http://www.state.nj.us/education/students/safety/health/records/athleticphysicalsform.pdfThe PPE form developed jointly by the American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP), American College of Sports Medicine, American Medical Society for Sports Medicine, American Orthopaedic Society for Sports Medicine, and American Osteopathic Academy of Sports Medicine, must be used for all student physical examinations prior to participation in school-sponsored interscholastic and intramural sports from this point forward.
The PPE includes a History Form to be completed by the parent or guardian; a Supplemental History Form for Students with Special Needs; a Physical Examination Form and a Clearance Form to be completed by the examining physician, advanced practice nurse (APN) or physician assistant (PA).

Sudden Cardiac Death In Young Athletes Pamphlet
Pursuant to N.J.S.A. 18A:40-41(d), school districts must distribute the Sudden Cardiac Death In Young Athletes pamphlet to each student-athlete and to the parents or guardians of the student-athletes, as part of the student’s pre-participation physical examination and completion of the athletic permission forms. A student-athlete and the parent or guardian of the student-athlete shall certify in writing that they received and reviewed the pamphlet.
